Winston-Salem/Forsyth County Purchasing, a division of Financial Management Services, obtains materials, supplies, equipment and service agreements and secures bids for materials and services for the City of Winston-Salem, Forsyth County, the City/County Utility Commission, and the Winston-Salem Transit Authority. The department ensures that local government gets the best equipment or service for the best price.

Independent Contractor Agreement

For the independent contractor agreement (DOCX) remember the following:

  • Use for non-professional service contracts (meaning service contracts that do not involve engineering, architecture, surveying, construction-manager-at-risk, design-build, or public-private construction services)
  • Use for the purchase of goods/materials/equipment/supplies
  • For pre-auditing purposes all departments must provide responsibility, object and/or program codes with the contractor's agreement. See Contract Checklist (fill-in onscreen form)
