Consolidated Annual Performance & Evaluation Report

The Consolidated Annual Performance and Evaluation Report (CAPER) reports accomplishments to Housing and Urban Development (HUD) for Community Development Block Grant, HOME Investment Partnership, and Emergency Solutions Grant program funds. The CAPER for the program year is available for review.  A CAPER is prepared and submitted to the United States Department of Housing and Urban Development each Fall, no later than September 28.

The Consolidated Housing and Community Development (CHCD) Plan serves as a single grant submission to Housing and Urban Development for Community Development Block Grant, HOME Investment Partnership, and Emergency Solutions Grant program funds and contains proposed activities and proposed expenditures within the context of the Five-Year Plan.

Public Notice of Substantial Amendment to FFY19 Annual Action Plan

Notice is hereby given by the City of Winston-Salem to amend the 2019-2020 Annual Action Plan of the Winston-Salem/Forsyth Housing Consortium to program additional funds provided by the U.S. Department of Housing and Urban Development (HUD) under the Coronavirus Aid, Relief and Economic Security Act (CARES Act). 

HUD announced $1,300,830 in Community Development Block Grant CDBG-CV funding for the City of Winston-Salem under the CARES Act to prevent, prepare for, and respond to coronavirus.  The City Council has approved $299,250 for a Diversion Program to help persons avoid emergency shelter entry or to exit quickly upon entry.  The $1,001,580 balance will be used to assist persons experiencing homelessness and meet supportive housing needs in the community.  Questions or comments about this substantial amendment may be directed to Mellin L. HUD also announced $2,047,257 in Emergency Solutions Grants ESG-CV2 funding for the City of Winston-Salem under the CARES Act to address the housing needs of homeless persons impacted by the COVID-19 pandemic and to prevent imminent homelessness for those impacted by COVID-19.  Funds may be used for rapid re-housing, homelessness prevention, and emergency shelter renovation or conversion.  Questions about this substantial amendment may be directed to Mellin L. Notice is hereby given that the 2019-2020 Annual Action Plan of the Winston-Salem/Forsyth Housing Consortium is amended. HUD announced $660,748 in Emergency Solutions Grants ESG-CV funding for the City of Winston-Salem under the CARES Act to address the housing needs of homeless persons impacted by the COVID-19 pandemic and to prevent imminent homelessness for those impacted by COVID-19. The City, as the Lead Entity for the Consortium, is required to amend the Action Plan in order to program the additional funds.
