Planning & Reporting

The Consolidated Housing and Neighborhood Services Plan (Con Plan) serves as a single grant application to HUD for a variety of funding sources that are used to support key services and projects in our community. The process also includes an annual action plan and performance report, to review progress toward comprehensive housing and service goals to outline plans for meeting those goals in yearly increments.

The 2024-2028 Five-Year Con Plan is ready for public comment and summarizes conditions and needs in Forsyth County as they relate to housing, community development, homelessness, and economic development and sets forth long-term goals and strategies to address those needs.

The Consolidated Plan (Con Plan) is a five-year plan, describing the city’s development needs, resources, priorities and proposed activities to be undertaken under certain U.S. Department of Housing and Urban Development (HUD) programs including:  Community Development Block Grant (CDBG), HOME Investment Partnership (HOME), Emergency Solutions Grant (ESG), and Housing Opportunities for Persons with AIDS (HOPWA). These federal funds are used to address affordable housing needs, homelessness, supportive housing services and community development needs for programs which either directly or indirectly benefit low- and moderate income households.

Consolidated Annual Performance & Evaluation Report

The Consolidated Annual Performance and Evaluation Report (CAPER) reports accomplishments to Housing and Urban Development (HUD) for Neighborhood Services Block Grant, HOME Investment Partnership, and Emergency Solutions Grant program funds. The CAPER for the program year is available for review.  A CAPER is prepared and submitted to the United States Department of Housing and Urban Development each Fall, no later than September 28.

Assessment of Fair Housing

The City of Winston-Salem, Forsyth County, and the Housing Authority of Winston-Salem (HAWS) completed an Assessment of Fair Housing (AFH and the AFH was accepted by Housing and Urban Development in November 2017. This study discusses fair housing issues and affordable housing needs throughout the county. It also outlines strategies the City, County, and HAWS plan to take to improve fair housing. The study is required by the United States Department of Housing and Urban Development for public housing authorities and jurisdictions that receive community development and affordable housing grant funds.

Annual Action Plan

Con Plan FY23 for Public Review identifies annual goals and proposed uses for Community Development Block Grant (CDBG), HOME Investment Partnership (HOME), Emergency Solutions Grant (ESG) funds, and Housing Opportunities for Persons with AIDS (HOPWA) funds.  The 2022-2023 Annual Action Plan contains actual allocation amounts for the 2022-2023 program year.


Funding Sources

Community Services Block Grant (CSBG)

The NSBG program provides annual grants to local governments to carry out a wide range of community development activities directed toward neighborhood revitalization, economic development, and improved community facilities and services.

Home Investment Partnershis Program (HOME)

The HOME program provides flexible grants to local governments for the purpose of increasing access to affordable housing for households with low and very low income.

Emergency Solutions Grants (ESG)

The ESG program helps individuals and families establish permanent housing after experiencing a housing crisis or homelessness. Grants are awarded to states and local governments to support homelessness prevention, emergency shelter and related services.
