If I fill in my address, will the person I’ve complained about be able to see who complained?

No, that person won’t be able to see your name or address. However, if someone reaches out to the city and files an open records request (through the Freedom of Information Act) the request would be reviewed by the city attorney’s office who would determine whether releasing that information is mandated by law.
