1. What are the guidelines to qualify to have a Mammogram?

Any woman ages 35 to 39 can have one screening Mammogram. Any woman ages 40 or older can have one Mammogram every 12 months.

2. Will my Blue Cross Blue Shield insurance pay for my Mammogram?

Yes, please complete the forms (Mobile Patient Information (PDF) and Breast History Form (PDF)). Send all of these completed forms as soon as possible to:

Please bring your insurance card and a photo identification with you to the screening. The Novant Health Breast Center mobile unit will file your insurance for you.

3. How do I schedule my Mammogram?

You can schedule yourself between Tuesday, November 10, 2015 and Wednesday, January 13, 2016. Simply call 336-718-7000. Please state that you are calling for the mammograms being done at the City of Winston-Salem. Don't forget to complete the required paperwork and send it to Employee Medical Services.

4. Can I have a Mammogram if my spouse works at the City of Winston-Salem, but I don’t?

Yes, if your name is listed on their insurance card or if you have your own insurance you can participate here at the City of Winston-Salem. Call 336-748-3866 if you have further questions.

5. Can I have a Mammogram if I don't have any insurance coverage?

If you do not have insurance and qualify according to the stated age limits, please contact Betty Speaks at 336-748-3866 or by email Betty Speaks. You may qualify for a Susan B. Komen scholarship.

If you have a strong family history of breast cancer, you can have your doctor make a referral for you to have a mammogram 10 years before your immediate family member was diagnosed.

6. What would disqualify me from getting a Mammogram at the mobile unit?

The following would disqualify you:

• If you do not meet the age requirements.
• If you are pregnant.
• If you have ever been treated for any breast problems.
• If you have already had a Mammogram within the previous 12 months.
