How do I use the program?

Self-Referral: Most requests for assistance will be initiated by an employee or family member directly by calling the EAP Assistance Phone Line (1-800-950-3434) 24 hours a day, 7 days a week.

Direct Referral: Your supervisor may refer you to the EAP if your job performance or conduct on the job seems to be affected by personal or job-related problems. In the event of a direct referral, information about your appointment will be relayed back to your supervisor only with a release of information from you.
